What Are Motiva Implants?

Motiva Implants® are silicone gel breast implants developed by Establishment Labs®, a global femtech company that has been pioneering breast augmentation technology for over two decades across more than 85 countries. The FDA approval covers two distinct implant styles: SmoothSilk® Round and SmoothSilk Ergonomix®. Both types of implants are supported by technology that differentiates them from conventional silicone implants.

SmoothSilk® Round

The Round implant pairs the biocompatible SmoothSilk® surface material with ProgressiveGel PLUS®, a silicone gel formulated to promote a soft, full upper breast. Round implants improve upper pole fullness and maintain their shape regardless of body position. They are well-suited for patients looking for well-defined projection.

SmoothSilk Ergonomix®

The Ergonomix® implant uses ProgressiveGel ULTIMA®, a softer, more adaptive gel that closely mimics the density and movement of natural breast tissue. What makes Ergonomix® unique is its ability to dynamically adapt to your positioning. The implant holds a round shape when lying down and transitions to a natural teardrop silhouette when standing. Both styles are available in Mini, Demi and Full projection.

What Is Motiva Preservé?

Motiva Preservé is an advanced, tissue-preserving approach to breast augmentation designed for patients who want subtle, natural-looking volume enhancement rather than a dramatic change in size. Where traditional augmentation techniques focus primarily on implant placement, Preservé is built around protecting the breast’s natural contours throughout the procedure.

The Preservé technique uses Motiva SmoothSilk Ergonomix® implants paired with specialized instruments and a pocket creation method designed to minimize trauma to surrounding breast tissue. The result is a procedure specifically suited to patients who are looking to enhance shape and fullness while preserving breast anatomy and, in Motiva’s stated design intent, breast function.

Because Preservé is a tissue-preserving augmentation technique, it is not a substitute for a breast lift (mastopexy). Patients dealing with significant drooping or sagging of the breast and nipple typically require a mastopexy, which involves repositioning the nipple-areolar complex and removing excess skin through a different, more involved incision pattern. Augmentation and mastopexy can be performed together, but they address different concerns.

Safety and Effectiveness Data

The nanotechnology behind SmoothSilk allows for microscopic variations in the surface of the implant, resulting in a much smoother and more realistic look and feel. Nano-scale engineering also makes SmoothSilk implants less likely to trigger an inflammatory response.

Clinical support behind Motiva continues to grow. The FDA’s Summary of Safety and Effectiveness Data (SSED) reported capsular contracture rates of less than 1% in primary augmentation patients and zero confirmed cases of BIA-ALCL, a rare lymphoma previously associated with aggressively textured implants.

Both the Round and Ergonomix implant styles use a form of ProgressiveGel®, a silicone formulation that mimics the density and movement characteristics of natural breast tissue. Implant Selection

Implant style, projection and placement are determined during consultation based on chest width, existing breast tissue volume, skin elasticity and your aesthetic goals. There is no single “right” choice, as the decision is anatomical and personal. Your AesthetiCare consultation and your surgical partner’s assessment will help inform and guide your decisions.

The Surgery

Breast augmentation with Motiva implants using the Preserve technique can be performed either under local anesthesia with oral sedation or general anesthesia, as an outpatient procedure lasting 1 to 2 hours. Incision placement is generally in the natural crease under the breast (inframammary fold) or around the nipple (periareolar). Thanks to the TrueMonobloc™ shell design, Ergonomix® implants can be inserted through a smaller incision.

Important Safety Information

Motiva SmoothSilk® Round and SmoothSilk Ergonomix® implants are FDA-approved for breast augmentation in women 22 years of age and older, including primary and revision augmentation. They are not currently approved for breast reconstruction in the United States. Recovery and Aftercare

Recovery from breast augmentation with Motiva implants generally follows the same timeline as conventional implant surgery, though the smaller incision may support a somewhat smoother initial healing window. Recovery is also impacted by the position of the implant, either on top of the muscle or under the muscle and this will be discussed at the time of your consultation.

Sun protection of incision sites is important throughout the healing period to prevent hyperpigmentation. Your surgical provider will schedule follow-up imaging as appropriate. Motiva’s clinical protocol recommends ongoing monitoring to ensure implant integrity over time. Patients who are non-smokers, in good general health and who commit to post-operative care instructions consistently achieve the best long-term outcomes.

Week 1

Expect soreness, swelling and tightness in the chest. Most patients are advised to rest, avoid lifting more than a few pounds and sleep in a slightly elevated position. Your surgical team will provide specific post-operative instructions; follow them precisely.

Weeks 2 to 3

Swelling begins to subside and most patients can return to light daily activity. Strenuous exercise, heavy lifting and overhead movements are still restricted.

Weeks 4 to 6

Implants begin to settle into their final position. Upper pole fullness may appear more pronounced early on before softening into the implant’s natural resting position.

Months 2 to 6

The full result continues to refine as swelling resolves completely and surrounding tissue adapts to the implants. Collagen remodeling continues during this period and scars fade progressively.

Frequently Asked Questions

How does the capsular contracture rate compare to traditional implants?

The U.S. IDE Study referenced in the FDA’s data reported a severe capsular contracture rate of 0.5% in primary augmentation patients. This figure is substantially lower than historical rates associated with traditional implants. Capsular contracture, which is the tightening of scar tissue around an implant causing distortion or discomfort, has historically been the primary driver of revision surgery. SmoothSilk® surface technology is largely responsible for the lack of inflammation and capsular contracture rates because of how its surface is engineered at the microscopic level.

What is the difference between Ergonomix® and Round implants?

The primary difference is gel formulation and positional behavior. Round implants (ProgressiveGel PLUS®) maintain a consistent, full shape regardless of body position, which is ideal for patients who want defined, predictable upper pole projection. Ergonomix® implants (ProgressiveGel ULTIMA®) use a softer adaptive gel that shifts with gravity: round when lying down and a natural teardrop when standing. Ergonomix® is designed for patients who prioritize the most natural-looking, position-responsive result possible. Both styles use the same SmoothSilk® surface, TrueMonobloc™ shell and BluSeal® barrier layer.

Am I a candidate for Motiva Preservé?

If you are looking for a subtle, natural-looking volume enhancement versus significant reshaping or lifting, you may be a candidate for this tissue-sparing approach. Patients with breast ptosis (drooping or sagging) who need the nipple repositioned or excess skin removed will typically require a mastopexy (breast lift), either alone or in combination with augmentation. What incision size is required?

Because of Motiva’s TrueMonobloc™ shell construction, Ergonomix® implants in particular can be inserted through an incision roughly half the size required for conventional implants. This reduces visible scarring, minimizes tissue disruption at the incision site and may support a smoother initial recovery. Exact incision size depends on implant volume, placement technique and your surgeon’s approach.

Is there any risk of BIA-ALCL with Motiva implants?

In the published U.S. IDE Study data, there were zero confirmed cases of BIA-ALCL (Breast Implant-Associated Anaplastic Large Cell Lymphoma) among Motiva patients. BIA-ALCL has historically been linked to aggressively textured implants. While no implant can claim a zero-risk profile for all complications, Motiva has been associated with a statistically significant drop in this complication.

How long do Motiva implants last?

Breast implants are not classified as lifetime devices, and Motiva is no exception. However, the clinical data demonstrates consistently low rupture rates over the study period and the ProgressiveGel® formulation is designed to maintain cohesivity over time. Many patients can expect their implants to remain in excellent condition for well over a decade with proper monitoring. Your surgical provider will recommend periodic follow-up imaging as part of your long-term care protocol.

What projection options are available?

Both Round and Ergonomix® styles are available in three projection levels: Mini, Demi and Full. Finding the right projection for any given patient requires a comprehensive review of chest wall dimensions, desired volume, existing tissue and aesthetic goals.
