As expected, the timing of these announcements has created a ripple effect throughout the industry. All compounded semaglutide and tirzepatide would be unavailable by May 22, 2025, which leaves patients with limited time to transition to FDA-approved alternatives or other options.

This regulatory shift represents more than just a policy change – now, hundreds of thousands of patients are stuck navigating uncharted waters as companies they entrusted with their health pursue strategies that may disrupt their care.

Why All the Controversy?

The popularity of compounded weight loss medications grew significantly during the shortage period, offering patients more accessible options when FDA-approved drugs were difficult to obtain. However, this accessibility came with trade-offs that are now becoming more apparent.

As of Feb. 28, 2025, the FDA has received more than 455 reports of adverse events with compounded semaglutide and more than 320 reports with compounded tirzepatide. These reports highlight potential safety concerns that have contributed to increased regulatory scrutiny.

The FDA also has received multiple reports of adverse events, some requiring hospitalization, that may be related to dosing errors associated with compounded injectable semaglutide products. Many of these issues stemmed from patients having difficulty measuring and administering the correct doses or healthcare providers miscalculating appropriate dosing schedules.

The unregulated nature of the compounding market created opportunities for questionable practices by some wanting to cash in on the new market. Many counterfeits became available online without a prescription and flooded the market, often made abroad and/or without state licensure, oversight or regulation.

The Reality for Patients Today

For patients who have been using safely using compounded alternatives, the current situation presents significant challenges. This puts them back at square one, struggling to afford the brand-name medications, which carry list prices of about $1,000 a month.

The financial burden represents just one aspect of the challenge. Many patients have built their treatment plans around the accessibility and affordability of compounded options, and the sudden shift requires rapid adaptation to new circumstances.

Some companies are attempting to continue offering compounded versions by claiming individualized patient benefits. Noom, a digital health company that prescribes compound semaglutide, the active ingredient in some of Novo’s GLP-1s, said it will continue to offer compounded versions of the drug beyond the May 22 enforcement deadline. 

However, pharmaceutical companies have pushed back against these practices, with one stating that anyone continuing to sell mass compounded tirzepatide, including by referring to it as ‘personalized,’ ‘tailored’ or something similar, is breaking the law and putting patients at risk.

Despite the challenges, patients are not without options. The medical weight loss landscape includes several FDA-approved medications beyond just the most well-known treatments. With a broader selection of weight loss medications now available, patients may ask what the strongest or most effective weight loss prescription medication is.

Healthcare providers must assess individual circumstances and recommend an appropriate course of action based on medical history, current health status, and treatment goals.
