Understanding Blepharoplasty

Blepharoplasty is a surgical procedure designed to improve the appearance of the upper eyelids, lower eyelids or both. This outpatient surgery removes excess skin, muscle and sometimes fat from the eyelid areas to create a more youthful and alert appearance.

Common Eyelid Concerns That Blepharoplasty Addresses

Drooping Upper Eyelids: Perhaps the most common concern that brings patients to consider blepharoplasty. This condition, known as ptosis when severe, occurs when the upper eyelid skin becomes loose and begins to hang over the natural eyelid crease. This can make the eyes appear smaller and create a tired, more aged appearance. In extreme cases, the drooping skin obstructs peripheral vision, making driving or reading more challenging.

Under-eye Bags: These bags form when fat pads that normally support the eyes begin to protrude forward, creating a puffy appearance beneath the eyes. This condition is often genetic and can occur even in younger individuals, though it typically becomes more pronounced with age. No amount of sleep or eye cream can eliminate true under-eye bags caused by protruding fat pads.

Excess Skin and Wrinkles Around the Eyes: The skin around the eyes is some of the thinnest on the body, making it particularly susceptible to the effects of aging, sun damage and repeated muscle movements. Fine lines and wrinkles that develop in this area can be improved through blepharoplasty.

Dr. Korentager brings over 40 years of exceptional experience in plastic and reconstructive surgery to our practice. As a board-certified plastic surgeon and Chair of the Department of Plastic, Burn and Wound Surgery at the University of Kansas, he represents the gold standard in surgical expertise and patient care.

What sets Dr. Korentager apart is his unique combination of technical precision and artistic vision. After earning his medical degree from the University of Toronto and completing specialized fellowships in breast reconstruction and micronerve surgery, he has dedicated his career to transforming lives through both reconstructive and cosmetic procedures.

The Blepharoplasty Journey

Consult

We begin with a detailed consultation to determine the best approach for your unique situation. During this consultation, we’ll examine the structure of your eyes, the quality of your skin and any asymmetries that can be addressed. We’ll also discuss your medical history and medications you’re taking to ensure you’re a good candidate for surgery.

Procedure Day

You’ll arrive at our outpatient surgical facility, where you’ll be prepared for surgery. The procedure is typically performed under local anesthesia with sedation.

For upper eyelid blepharoplasty, incisions are made along the natural crease, making any resulting scars virtually invisible once healed. Excess skin, muscle and sometimes fat are carefully removed or repositioned for a more youthful contour. The incisions are closed with fine sutures that will be removed within a week of surgery.

Lower eyelid blepharoplasty can be performed through different approaches depending on your needs. If only fat removal is necessary, the procedure may be performed through an incision inside the lower eyelid (transconjunctival approach), leaving no visible scar. If excess skin also needs to be removed, it’s taken care of with an incision just under the lower lash line, and the scar typically heals barely visible.

An entire procedure usually takes one to three hours, depending on whether your upper lids, lower lids or both are treated. Most patients go home the same day, though you’ll need someone to drive you and stay with you for the first 24 hours.

Recovery and Healing

The recovery process following blepharoplasty is generally straightforward, requiring patience and careful adherence to post-operative instructions.

Immediately following surgery, you’ll experience some swelling and bruising around the eyes. Your eyes may also feel tight and somewhat uncomfortable, and you might experience some sensitivity to light. Cold compresses applied gently help reduce swelling and provide comfort during the first few days.

In the first week of recovery, it’s important to keep your head elevated while sleeping and to avoid activities that could strain your eyes or increase blood pressure. This includes avoiding heavy lifting, strenuous exercise and bending over. You’ll need to avoid wearing contact lenses for at least two weeks and may need to use prescription eye drops to keep your eyes properly hydrated.

The stitches from upper eyelid surgery are typically removed within five to seven days, while lower eyelid sutures may be removed later. Most patients return to work and normal daily activities within seven to ten days, though you may still have some residual swelling and bruising that can be concealed with makeup if desired.

Complete healing and the final results of your blepharoplasty may take several months to become apparent. While the most dramatic improvements are visible within the first few weeks, subtle changes continue as the tissues settle and any remaining swelling subsides. The scars will continue to fade, eventually becoming barely visible thin lines that blend naturally with your skin.

Long-Term Results and Maintenance

One of the most appealing aspects of blepharoplasty is the longevity of its results. The excess skin, muscle and fat removed during the procedure will not return, giving you years of enhanced appearance.

Your skin will continue to age, and you may eventually develop new signs of aging around the eyes. The good news is that you’ll always look better than you would have without the surgery, and many patients find that their results last ten to twenty years or even longer.

To maximize the longevity of your results, maintain a good skincare routine that includes sun protection, moisturizing and quality eye creams. Avoiding smoking, excessive sun exposure and other factors that accelerate skin aging will also help preserve your results.

Combining Blepharoplasty with Other Treatments

While blepharoplasty can achieve remarkable improvements, many patients combine their eyelid surgery with other treatments for more comprehensive facial rejuvenation.

Ideal Candidates for Blepharoplasty

Blepharoplasty can dramatically improve the appearance of the eyes, but it cannot stop the aging process or completely change your fundamental facial structure. The ideal candidate for eyelid lift surgery is someone who is bothered by the appearance of their eyelids and has realistic expectations about what the procedure can achieve.

A strong candidate will have excess skin on the upper eyelids that may be impairing vision or creating an aged appearance, noticeable bags under the eyes that don’t improve with rest, fine lines and wrinkles around the eyes that bother you and good overall health with no serious eye conditions or bleeding disorders.

Most patients are over 35, though some with genetic predispositions to under-eye bags may benefit from the procedure earlier.

Individuals who are not good candidates for blepharoplasty include those with certain medical conditions that impair healing, people with dry eye syndrome or other serious eye conditions. These issues may need to be addressed before considering surgery.
